Case Management
Case management is a critical component of any supported living program. Treatment team members will develop and supervise the implementation of the residents individualized treatment plan, ensuring that the plan incorporates acquisition of social skills, activities of daily living, vocational endeavors, medical management, behavioral management, and recreational activities. Case management will monitor and revise the treatment plan as service needs change over time. The Program Director/Designee will develop a discharge plan, if applicable, for residents in coordination with the resident, family, funder and other stakeholders.
Program Costs
Communicare offers a comprehensive per-diem rate that is negotiated based on individual resident needs for support, supervision, and daily assistance. The comprehensive per-diem rate includes, but is not limited to the following services:
A. Support, supervision and care 24 hours a day to maintain health, safety and well-being of the resident. (Personal care attendant and/or one-to-one supervision are not included.)
B. Room and Board, which includes a weekly budget for groceries, recreation and leisure activities.

D. Social Skills Acquisition Training
E. Basic Meal Preparation and Safety
F. Linen and bedding.
G. First aid supplies.
H. Standard case management services.
I. Monitoring and teaching activities of daily living.
J. Daily activity schedule based on individual treatment plan.
K. Organized leisure and recreational programs.
L. Admission health assessment.
M. Transportation related to program services.
N. Teaching of the self-government process for home management.
O. Family/significant other behavior management training.
P. Sheltered Workshop
Q. Adult Basic Education
R. Assistance with Medicaiton Administration / Medication Training
